A Universal Definition for a Global Problem

Heart failure is a global health problem, but how it is defined, diagnosed, and classified has not always been consistent. Differences in terminology and diagnostic criteria can affect everything from how patients are diagnosed to who gets included in clinical trials. A new international consensus seeks to create a common language for understanding heart failure across different health systems and populations.

Why Heart Failure Needed a New Definition

This Second Universal Definition of Heart Failure was developed jointly by the American Heart Association, American College of Cardiology, European Society of Cardiology, and World Heart Federation. The goal is straightforward: create a standardized framework that reflects changes in how heart failure presents, how we diagnose it, and what we now understand about the underlying disease.

This matters because heart failure itself is changing. Populations are aging, heart failure with preserved ejection fraction is becoming more common, and advanced imaging and genetic testing allow clinicians to identify disease that might previously have gone undetected. At the same time, the causes of heart failure vary significantly around the world. Ischemic heart disease predominates in some regions, while hypertension, rheumatic heart disease, Chagas disease, and other conditions play much larger roles elsewhere.

Beyond a Single Heart Test: Rethinking Diagnosis

Access to diagnosis and quality treatment also varies considerably. Geography, income, race, ethnicity, sex, and access to healthcare all influence heart failure risk and outcomes. A universal definition cannot eliminate these disparities. But creating a common framework for identifying and classifying the disease should make it easier to understand where disparities exist and how outcomes differ. The most significant changes emphasize the dynamic aspect of the disease and moves away from ejection fraction cutoffs. 

What This Means for Patients and Care Disparities

Medicine advances when we can reliably identify what we are studying. A common definition gives clinicians and researchers around the world a shared starting point. And better defining who has heart failure, why they have it, and where they are in the course of disease should ultimately lead to earlier diagnosis and better care for individual patients.
